During the Start of Care visit, a licensed Registered Nurse will conduct a comprehensive assessment of you or your family member’s needs, consult with your Physician and create a customized Plan of Care with your participation and preferences. Ocean-1 Home Health Agency will coordinate your care with all specialty services required to meet your health needs.

Who is qualified for Home Health Care?

- Patients that require home health services in their places of residence and are under the care of a physician that certifies their need for skilled care from a professional Nurse or Physical/Occupational/Speech Therapist.
- Patients that participate in Medicare/Medicaid programs or other qualifying health insurances, that have a serious illness or injury and meet the criteria of being home-bound, use a supportive device such as a crutch, cane, wheelchair, walker, inability to leave home without assistance, or it is medically contraindicated to leave home.
- Private paying patients that require home health care services.

Registered Nurse will provide a comprehensive assessment of the patient, and create a customized plan of care with the assigned multidisciplinary team, the physician, patient, and family to ensure optimal care

Skilled Nurse provides treatment and care, ie; wound care, intravenous medication, injections, etc. as per the physician's orders

Physical / Occupational / Speech Therapy provides treatment for post-surgical conditions like Total Hip or Knee Replacements, Strokes, Post Trauma, etc

Medical Social Worker will assist with community resources and personal counseling

Certified Nursing Assistants /Home Health Aides provides personal care which includes Bathing, Safe Transfers, Range of Motion Exercises, Medication Reminders, etc

Ensure the BEST care possible for the patient in the privacy and comfort of their own home
